Intake Procedure

The procedure to apply for our service is as follows:

  1. Fill in an application form
  2. A visit to the patient in the hospital or at home by one of our managers. A medical assessment will be done with recommendation from Care@Home’s Registered Nurse/Staff Nurse.
  3. Presentation of a quotation to the patient or his/her representative.
  4. Consequent confirmation of the quotation.
  5. The care manager will deal with all negotiations regarding fees.
  6. Drafting, approval, and signing of the care plan.  The latter will stipulate the type of service to be rendered, tasks, days and hours involved and date of commencement.
  7. The signing of the final contract between Care@Home and the patient and/or representative.

What about medical aid funding?

  • It is the responsibility of the client or the client’s family to get the authorisation from their medical aid.
  • We do not work directly with any medical aid services.
  • The client will be responsible for all payments.
  • Claims can be done from the medical aid afterwards.
